Games are played on a standard basketball floor, however, the main distinction is that contact is permitted. It is played on a padded court, and participants wear elbow and knee pads to prevent injury.

There are free throws for shooting fouls and a three-point line, but there is no traveling in wrestling. The game also begins with wrestling to determine who will have possession rather than a jump ball. The sport has grown in popularity in Russia, and there is even a professional competition called the International Wrestball Competition.

Teams can have up to six players on the floor, but a maximum of 13 players can participate. The game does not include dribbling, which is the act of bouncing the ball while running. Players are also not allowed to use helmets, knee pads, or elbow protection, though they are typically declined. The sport, like most mental things, has its roots in Russia, where a professional league consisting of 22 teams plays it. It's regarded by many as one of the hardest and most physically taxing sports available.
